RQI Problem Category Definitions

Nursing and Personal Care includes
- assessment and reassessment of residents
- management of pain and pressure sores
- weight change management
- general nursing care

Care Plans includes
- implementation, accessibility, reviews and revisions as required, interdisciplinary care conferences which include family
- documentation of care plans is current, complete and accurate

Resident Rights includes
- residents’ rights to dignity, privacy, respect, individuality, and freedom from mental and physical abuse; communication amongst family and staff; consent to treatment from resident/family
- appropriate resident activities and programming
- bathing/grooming provided as appropriate

Restraint Use includes
- resident/family consent, doctor’s order, repositioning, required documentation, etc.

Medication includes
- medications ordered and signed for by appropriate personnel, storing, documenting, dispensing and evaluation of medication use

Dietary includes
- issues related to nutrition, adequacy of food and fluids, and meal service with staff support as necessary

Environmental Safety/Hazards includes
- infection control
- any practice that could contribute to risk or injury, such as water temperature safety, poor disaster plans, unlocked doors

Maintenance Issues includes
- maintenance, housekeeping ,personal laundry maintenance, and general cleanliness
- unclean or inadequate linen and supplies
- offensive odours

Facility Management includes
- staffing issues
- information provided to residents
- critical incident reporting

Quality Assurance includes
- professional development
- abuse and other policies
- satisfaction surveys
- program evaluation

Resident/Family Council includes
- timely, written response to concerns
- supporting, consulting with councils as required
- providing relevant information
